JOB TITLE: Aging Specialist
LOCATION: Petoskey, MI
REPORTS TO: OBRA Supervisor
STATUS: Non-Union, Full time
HYBRID ELIGIBLE: Yes
STARTING WAGE RANGE: $58,718 - $61,069 per year
QUALIFICATIONS:
Education: Master’s degree in an applied area of social work, psychology, or counseling. Must meet the requirements for Licensed or Limited Licensed Master Social Worker, Licensed or Limited Licensed Professional Counselor, or Licensed or Limited Licensed Psychologist.
Experience: Possess experience in clinical social work with the mentally ill, with expertise in mental health and aging, developmentally disabled individuals, persons with dementia, or other relevant human services work.
Other: Lived experiences with behavioral health issues are valued.

SUMMARY OF RESPONSIBILITIES:
The Aging Specialist participates as a member of the multidisciplinary team, providing comprehensive pre-admission screenings (PAS) and annual resident reviews (ARR) to persons identified via the OBRA screening process. Provides CMH and nursing home consultation, staff education and training, and direct treatment services, including individual therapy and mental health monitoring.

ESSENTIAL JOB FUNCTIONS:
  • Assesses individuals referred for Level II OBRA screenings and the Older Adult Services program.
  • Completes reports, paperwork, and communications needed to finalize OBRA evaluations.
  • Participates as the Aging Specialist in the nursing home consultation and training program.
  • Provides therapy, monitoring, and coordination of services to OBRA/Older Adult Services identified consumers residing in nursing homes in accordance with the plan of service.
  • Acts as a liaison for other CMH programs by assisting with referrals to and from nursing homes or alternative placements.
  • Is a part of the NCCMH treatment team assisting in the treatment planning and appropriate placement of elderly NCCMH Consumers.
  • Requests specialized services from NCCMH as needed.
  • Provides appropriate clinical documentation for all services provided.
  • Fulfills the elements of the nursing home CMH agreements.
  • Informs supervisor of new developments within the nursing home programs.
  • Documents activities in the paper or electronic clinical record and the agency information management system in accordance with agency policies and Medicaid requirements.
  • Meets direct and indirect service activity expectations.
  • Participates in NCCMH Emergency Services “On Call” rotation.
  • Perform other related duties as assigned by supervisor.
PHYSICAL REQUIREMENTS:
Ability to walk, bend, stand, sit, lift to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in CPI techniques which will be a training class provided to the job incumbent. OTHER REQUIREMENTS:
  • Must be current in Adult CPR and First Aid; provided by NCCMH.
  • Complete initial training in Non-Violent Crisis Intervention (CPI); provided by NCCMH.
  • Have own phone for communication, reliable access to messages and multi-factoring capabilities.
  • Have reliable transportation in carrying out agency duties.
  • Possess a valid Michigan driver’s license and have a satisfactory driving record.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
  • Supports a philosophy of service delivery that is recovery-based, person centered, and culturally competent.
  • Adhere to applicable policies and procedures as required by the Department of Community Mental Health, the Mental Health Services Board, and other pertinent regulatory agencies.
  • Proficient with Microsoft Windows and the Office Suite, especially Outlook, Word, and Excel; able to maneuver on the Internet to conduct research or obtain information, including EHR and Video Conferencing.
