Assistant Coordinator of Community Habilitation Services (903)

Roosevelt, NY

Shift: Monday – Friday, 40 hours/wk

Salary Range: $48,000 – $50,000/yr

Qualifications:

• Associate’s Degree preferred
• Clean and valid NYS Driver’s License required
• HS Diploma required
• 3 years of experience working with individuals with developmental disabilities required
• Computer skills

Responsibilities include, but not limited to:

• Ensure that quality person centered services are provided to each individual participating in the program.
• Participate in matching process between the person with disabilities and the caregiver.
• Work closely with the MSC ensuring in program services are well coordinated services.
• Mediate issues between the individual with disabilities and the caregiver.
• Participate in training
• Participate in the 24-hour emergency cell phone rotation.
• Monitor and oversee tracking and review of outcome measures.
• Provide documentation as per OPWDD and DOH requirements.
• Evaluate the performance of all staff and responsible for the safety of all staff under direct supervision.
• Department related travel as required.
• Write, develop and monitor ADL/ILS goals in compliance with state and federal requirements for active treatment programs.
• Work with individual residents and the IDT regarding scheduling, planning, implementation and supervision of all individuals’ active treatment in the form of ADL/ILS goals.
• Participate in the ISP meeting regarding the individual habilitation plans.
• Instruct and consult with direct care staff regarding their participation and role in various programming.
• Review and update, annually and as needed, the Comprehensive Functional Assessment. Assure all evaluations are updated.
• Maintain a high level of confidentiality at all times.
