The Supv, Transition of Carewill assist the department director in the coordination of the daily operations of the bedside RN Case Manager and Social Work team.This includes the coordination of services to meet patient psychosocial needs, assuring a cost and quality effective discharge, promoting patients' rights, and maximum benefits from healthcare.
EXPERIENCE: Supervisory experience, five years in the medical field: i.e. hospital, home health or nursing home.
LICENSE/CERT: Preferred: CCM (Certified Case Management)
EDUCATION: Required: Masters - Social Work (CSWE approved) or Bachelors - Nursing
