Overview:
The Emergency Services unit is available 24/7 to provide on-site or mobile assessments and referrals for further treatment to any individual experiencing a mental health or substance abuse problem, or both. The ESP services communities within Worcester County. Services are provided to adults in various community settings and at emergency rooms within the catchment. Our staff assess clients for critical symptoms and work with the client to coordinate their next phase of treatment. The Emergency Services Program uses a strength-based and person-centered approach when intervening in crisis situations to ensure that clients are provided individualized care and support. Case Manager
in Worcester, MA.

- Provides support and guidance to adults experiencing crisis.
- Dispatch into the community with a clinician to support AMCI clinicians as well as YMCI clinicians as needed.
- Assists in the development of a risk management/safety plan.
- Refers clients to support services.
- Acts as a bridge between the client, agencies, and community supports.
- Assures clients' full involvement in crisis interventions services including team's adherence to "client voice and choice"
- Be an active participant in the multi-disciplinary team meetings and case discussions. Recognize and seek consultation as needed.
- Develop a training plan to enhance ongoing growth.
Provide consultation and direction to AMCIs to promote collaboration with the client's care team. - Provide consultation to AMCI staff regarding work with clients.
- Assist with providing follow up and bed searching to promote overall functioning of both AMCI and YMCI programs.
- Develop ongoing client support resources.
- Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
- Grade C04: Case Manager II: Requires a Bachelor's degree; in Psychology, Counseling, or Social Work with 1 year experience working in the field.
- Bilingual/Spanish highly desired.
- Knowledge of substance use disorder, mental health and related issues, familiarity with community resources and entitlements.
- Strong analytical skills, including the ability to recognize problems, think creatively, and devise innovative solutions.
