Position Summary: Reporting to the City Center Program Manager, the Case Manager will assist residents in accessing care and services, advocate for resident needs, and aid in removing barriers to accessing permanent housing. The Case Manager will assist in mapping out plans that include linkage to identified available services and provide ongoing monitoring of resident progress toward completion of stabilization, income, and housing goals. The ideal candidate enjoys working as part of a team of staff and volunteers and can think and act quickly when there is a need to pivot or address an emergency. This candidate can think critically and help residents determine a course of action that will move them toward stability. This individual can work independently while ensuring complete and thorough communication with team members. The best candidate for this post is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow through from residents. This representative must be energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations. This person will exhibit the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment. The goal of this position is to provide shelter residents with the tools and support required to permanently end their homelessness. City Center is a transitional housing shelter extension program established in partnership with the City of Phoenix. The goal of City Center is to provide graduated housing opportunities for residents moving from a congregate shelter setting and working toward permanent housing solutions. City Center is located in a hotel that has been converted for use as a transitional shelter. This is a round the clock closed shelter program.


Requirements:

  • Level One Fingerprint Clearance or ability to obtain within 90 days of employment.
  • CPR and First Aid certification or ability to obtain within 90 days of employment.
  • Excellent communication, organization skills, and ability to interact with residents, staff, and volunteers required.
  • Driver’s License and clean 39 month driving record will be shown preference.
  • Related experience working with vulnerable populations, volunteers, and staff in a temporary shelter environment preferred. Knowledge of available community supports and services is beneficial and will be shown preference.

ADA Requirements: Abilities to stand and walk, lift up to 50 pounds, sit, bend, and reach are required for this position. Ability to read, comprehend, and communicate orally and in writing required. Strong computer skill, proficiency with Microsoft Suite programs, and excellent electronic communication and documentation preferred. Accommodations may be made available if need is expressed.

Specific Duties:

  • Carry a caseload of up to 25 shelter residents, meeting with each resident a minimum

of once per week.

  • Complete intake with residents and assess current needs to create a housing plan and individual service plan.
  • Ensure that complete and accurate electronic records of resident encounters, social

supports, and agency referrals are generated within 24 hours of interaction in HMIS.

  • With resident input, monitor progress through care plans, goal sheets, and service referrals.
  • Participate in weekly case conferencing to monitor resident progress on goals and service plans.
  • Complete monthly caseload report for Shelter Program Manager.
  • Coordinate incident intervention with other staff and security when needed.
  • Aid in conflict resolution through groups, mediation, or individual coaching.
  • Coordinate or provide transportation to appointments or services for residents as needed.
  • Assist residents to move into permanent housing.
  • Provide street outreach and assessment to prospective program residents when needed.
  • Attend staff meetings, trainings, and agency events as requested or required.
  • Other duties as identified by Shelter Management.
  • Participate in and serve as a model for SVdP’s Culture of Philanthropy and ensure that SVdP volunteers feel welcomed and appreciated.
