Home Instead is looking for a competent and professional Client Care Coordinator Manager to join our team!

In this role, you will be working closely with clients and Caregivers alike to ensure quality care is provided.

To be qualified for this role:

• Candidate must have medical or nursing background (RN, LVN);
• Candidate must have caregiving experience, or any similar capacity;
• Candidate must have at least 3 years of people-managing/ supervisory capacity role;
• Candidate must be knowledgeable of payor sources (insurance, Medicaid, Medicare);
• Candidate must be able to apply interviewing skills (active listening, open and close-ended questions, supportive responses) to clients to assess rooms for improvement and necessary intervention steps.
• Candidate will be working in the office.

Job responsibilities:

• Develop, maintain, and overview Client Care and Quality Assurance program for the Home Instead office.
• Maintain a current plan of care outlining the specific services to be provided to client. Ensure proper documentation and update regularly.
• Conduct quality assurance home visits with clients on a regular schedule.
• Provide oversight, up to and including the instruction of initial and ongoing in-office and field training for staff.
• Gain mastery of the following systems/ tool: WellSky (ClearCare), Empower
• Manage and lead the team of Caregivers by spearheading training (CEUs); Client Care delegation to ensure highest standard of care is given to clients.
• Available for regular on-site supervision of the provision of services by Caregivers as needed to ensure quality of care, client safety, and meet state standards.
• Report directly to franchise owner.
• Other ad hoc tasks that may be assigned.

Job Type: Full-time

Pay: $65,000.00 - $85,000.00 per year
