CLASSIFICATION/STATUS: Exempt, Full Time, Permanent

IMMEDIATE SUPERVISOR: Behavioral Health Director

SUPERVISORY RESPONSIBILITIES: None

FLEXIBLE WORK: Hybrid schedule option

PAY RANGE: $26.50- $37.00/hr.

YOUR ROLE & IMPACT

The Family Partner brings a key family centered perspective to the primary care team and is a subject matter expert in child and adolescent mental health and/or substance misuse, parental mental health and/or substance misuse, navigating systems, and understanding and breaking down barriers to treatment. The Family Partner will facilitate information sharing and care in a member focused way. The Family Partner will clarify communication about symptoms, diagnoses and treatment planning, and will enhance engagement between parents and clinicians, so that children and youth are more likely to receive needed health services. This role will be the primary and consistent contact for outside agencies such as the Child Children’s Behavioral Health Initiative (CBHI), state agencies, mental health agencies and schools.

YOUR RESPONSIBILITIES

  • Identify support/services for children and families based on the evaluation of the child’s needs.
  • Coordinate resources, service recommendations and appropriate care.
  • Communicate family/child’s needs with the integrated clinical team, family, PCP, school administrators/personnel and external providers.
  • Attend regular meetings with physicians and other staff at the health center, to share concerns on behalf of families who may have missed an appointment, not connected with a referral or who are otherwise at-risk.
  • Assist with parent/family communication during initial visits, as well as any follow-up visits
  • Follow-up with identified families, either at the clinic or in their homes, at the request of the clinical team, regarding the child's mental health status or treatment needs.
  • Track and follow up on referrals to ensure families connected with services.
  • Clearly and effectively, document information and activities in the EMR.
  • Engage in providing or coordinating evidence-based screening of children and adolescents for mental, behavioral and developmental health conditions, including depression, anxiety, unhealthy alcohol and drug use.
  • Use a variety of educational materials, brief intervention techniques, and community resources to engage patients and support families in coping with mental health symptoms.
  • Communicates with outside agencies that may be involved with the child/family including but not limited to CBHI, School Systems, DYS, DCF and/or DMH.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • MSW (Masters of Social Work) and current Massachusetts licensure in Social Work (LCSW); or ability to obtain one within 3 months of hire required.
  • Related experience advocating for children and families and collaborating with clinicians and/or other professionals.
  • Strong knowledge of child mental health treatment and resource options.
  • Strong knowledge of child/adolescent systems (CBHI, School, DCF).
  • Problem-solving and behavioral change strategies appropriate to children and families.
  • Ability to represent "family voice" on the clinical and leadership teams.
  • Ability to rapidly assess patient’s needs.
  • Ability to communicate clearly and effectively with families, medical professionals, behavioral health professionals and school administrators and personnel.
  • Ability to provide accurate feedback about alcohol/drug use without judgment or discomfort.
  • Ability to maintain a professional stance if conflicts arise with other staff.
  • Must be able to work 2 days in Brighton and 3 days in Waltham location
