The Intake Coordinator coordinates and completes intakes for all chemical health outpatient and family residential programs. This position assists in client account setup for care coordination, case management and housing programs. This role will also follow up with referral and funding sources prior to, during, and after client admissions.

RESPONSIBILITIES: *

  • Complete Intake paperwork’s with new residents to Avivo Village.
  • Help to explain/complete all paperwork as it relates to intake for both Avivo Village and Street Outreach programs.
  • Coordinate any necessary documentation/paperwork with the appropriate individuals at intake.
  • Complete all admission paperwork and ensure proper entry into the corresponding databases and workflows.
  • Ensure services provided are culturally responsive utilizing best practices.
  • Perform clerical work such as filing, copying and uploading.
  • Assist with billing functions, including data entry and insurance information.
  • Provide information on Avivo services to individuals seeking services, including in person and on the phone.
  • Provide general administrative assistance with data entry, general reports, table and document development, typing and editing.
  • Actively participate in Avivo’s diversity efforts and committees.
  • Actively participate in Avivo’s trauma informed care initiative.
  • Identify, develop, share, and incorporate best practices and strategies within the division.
  • Maintain participant files; submit reporting forms, and other data as required.
  • Positively represent the organization, program, and participant's needs.
  • Attend and participate in regularly scheduled staff, agency, and external meetings.
  • Follow program operations according to program plans and goals in accordance with contract requirements.
  • Keep immediate supervisor informed of all issues of significance to facilitate prompt action as appropriate.
  • Perform miscellaneous projects and complete varies tasks as assigned.
  • High school diploma or equivalent (GED).

  • Experience and desire to work with persons of diverse cultural, economic and educational backgrounds.
  • Competency in MS Office Suites, e-mail and Internet.
  • Familiar with grants, funding sources and quality programs and services for persons with barriers.
  • Self-motivated, show initiative, and able to work independently.
  • Ability to maintain confidential information and successfully complete Health Insurance Portability and Accountability Act (HIPAA) test.
  • Possess basic math skills for the maintenance of budgets, P&L and reporting purposes.
