Keystone Health, Idaho's leading Housecall provider since 2013 and located in Eagle, is currently searching for a Social Worker to join our Chronic Care Management team. The ideal candidate is detail-oriented, organized, compassionate, reliable, and a team player. This position involves discharge planning, resource development, admissions, and orchestrating seamless care for our patients while working from our office location in Eagle. The CCM program collaborates directly with providers in a variety of healthcare settings to identify patients with chronic conditions and alleviate gaps in care. Chronic Care Managers interact with patients and families via phone calls, emails, and visits to ensure comprehensive care.
