Job description
A New Behavioral Health agency, Braylon Health Wellness Center is seeking a Case Manager. This position provides assessment, planning, implementation, and overall evaluation of individual client needs for mental health and/or substance use disorder (SUD) services. The Case Manager provides clients and their families with ongoing community support resources including transportation for access to identified resources.
Reporting to : Clinical Director
Up to $55,000 annual salary based on experience
Up to $1000 Sign-On bonus
Roles and responsibilities:
1. Provide support in accessing and developing a sober support network.
2. Collaborates effectively with a client's treatment team.
3. Provide support in accessing and developing a sober support network.

5. Maintain client charting
6. Assist in crisis intervention and prevention in the community.
7. Participate in community outreach.
8. May be required to perform tele-health services as determined by Braylon Health.
9. Perform any other duty as required or demand
10. Achieve productivity goals as set.
Requirements:
A) Education: High School or Better
B) Experience:
C) Licensure: Valid unencumbered Ohio driver's license and proof of driver's insurance are required.
D) State: Ohio
Requirements: Preferred
Experience: 1-3+ years of previous case management or relevant experience
ii) Knowledge of the community, community organizations, and community resources.
C) Licensure: LSW or LISW
