What does a Transition Support Specialist at Prospect Hill do? The Transition Support Specialist provides students with instructional opportunities and experiences designed to support students on their pathway to graduation and readiness for their chosen post-high school goals. The teacher will plan and implement whole and small group instruction that is individually designed to focus on transitional topics to prepare students for college and career readiness. The teacher will work closely with the students case manager, instructional team, and related service providers to create a supportive learning environment in which learning and engaging in transition work is an ongoing and positive experience. The transition support specialist will support students who require alternative pathways to credits and graduations such as credit recovery options, online classes, and supported independent study. A successful candidate will identify with PHAs mission and have a demonstrated commitment to providing individualized support to students with a broad spectrum of need while maintaining high expectations and centering the students voice and goals.
Essential Responsibilities:
- Plan and implement a structured learning environment including explicit instruction in executive functioning skills needed to help students engage in and progress through individualized credit recovery programs.
- Collaborate with members of students teams to plan and implement vocational/transitional experiences as indicated in student IEPs.
- Develop educational goals and objectives for individual student IEPs; participate in team meetings
- Document student progress on IEP goals and transition plans and report on progress notes as required
- Generate and maintain partnerships and collaboration with community based sites at which students can engage in transitional/vocational experiences
- Plan and implement structured study skill experiences to support students in staying engaged with high school classes required for graduation
- Maintain a professional level of knowledge related to supporting students with transition related goals and experiences including participation in professional development opportunities
- Fulfill non-instructional duties as assigned by the building director (morning supervision, advisory, bus duty, etc.).
- Maintain a high degree of professionalism at all times (attendance, punctuality, dress, written and oral communication, etc.).
- Perform other duties assigned by the Director of Student Services and/or the building Principal.
- Bachelors Degree required, Masters Degree preferred
- Current Massachusetts licensure in Special Education (Teacher of Students with Moderate Disabilities, K-8 or 5-12) OR Transition Specialist Endorsement OR experience supporting students with alternative pathways to graduation and/or with vocational and community based experiences
- Comprehensive understanding of state and federal regulations pertaining to Special Education
