Job description
The primary function of the Community Inclusion Specialist is to implement pre-vocational/career services aimed at assisting individuals to achieve their highest level of independence. The Community Inclusion Specialist ensures participants take full advantage of the job readiness curriculum and other employment preparation activities. The Community Inclusion Specialist interacts with employers in the community to ensure the employer has a quality experience in working with individuals with developmental disabilities. The Community Inclusion Specialist must document services both thoroughly and timely; assesses and monitors outcomes to relay to the team to ensure the participant is as independent as possible.
Duty to Inform: Responsible to promptly report to supervisor, and up the change of command, in the absence of the supervisor, any circumstances that negatively affect the health, safety, or well-being of residents, staff and the agency/residents/staff/property/equipment.
- Implement curriculum to provide a positive learning experience for participants. Encourage individual empowerment through choice making. Plans, develops and implements strategies for developing individual’s independent work skills. Assists, monitors, and supports individuals in all areas of activities including but not limited to:
1. Hygiene
2. Budgeting
3. Communication
4. Social skills
5. Community Integration
6. Peer Interaction
7. Age appropriate instruction / environment
8. Emergency Skills
9. Computer
10. Skill Development
11. Senior Curriculum (Current Events, Recreation, Socialization)
Summary
Essential Duties and Responsibilities include the following; other duties may be assigned: Accompanies individuals to job sites; observes performance; oversees work training; provides job retention training, instruction and assistance accordingly.
- Provides support to Individual and team members in obtaining participant’s information to assess employment history, education, skills and abilities and potential barriers to employment. Functions as an advocate and customer service liaison between site personnel and the individuals to maintain open communication and achieve community inclusion goals. Ensures adherence to HIPAA standards at all times.
- Is a continuous positive role model for appropriate work behavior, work attitude and work ethic, and is expected to conduct themselves accordingly at all times, including wearing appropriate work attire for job site.
- Facilitates daily program activities, maintains attendance logs and any other required documentation. Provides individual and/or group academic assistance to participants
- Monitors activities of individuals during assigned activities to ensure physical and environmental safety, or for the purpose of reporting on critical/behavioral incidents.
- Maintains progress notes on participants’ outcomes; collects and completes all necessary documentation and reports related to program activities and submits timely.
- Reports all Unusual and Major Unusual incidents reports according to agency policy/procedure.
- Participates in investigations as required per policy/procedure.
- Conducts and documents fire drills, as required.
- Conducts and documents emergency drills, as required.
- Participates in all regulatory and compliance reviews, as required.
- Implements individualized programs that will ensure participants attain and maintain community inclusion as per the individual’s person centered planning.
Job Type: Full-time
Pay: $14.00 per hour
Schedule:
- 10 hour shift
- 8 hour shift

- Cleveland, OH: Reliably commute or planning to relocate before starting work (Required)

- Caregiving: 1 year (Required)
