Education & Training: Bachelor of Science in Nursing required. MSN preferred.

Experience: 3+ years Case Management or discharge planning experience preferred. 2+ years Supervisory/Management experience required.

License, Certification & Registration: Current licensure as Registered Nurse in the Commonwealth of PA.

Other Requirements: Excellent verbal and written communication skills. Ability to interact professionally with both internal and external customers. Basic computer skills required.

Primary Function:

Under the direction of the Director Care Coordination Management, the Manager of Care Coordination Management/Social Work Department is responsible for managing the daily activities of the R.N. Care Coordination Managers. In conjunction with the Director, the Manager of Social Work and the Medical Director of Utilization Review, ensures compliance with all Medicare regulations, other pertinent regulatory agency regulations and commercial payer contracts for the Department. This role ensures that all care coordination management and utilization review processes are consistently applied. The Manager Care Coordination Mgmt serves as a resource to the care coordination managers, the social work staff, and office staff.
