Accurate data integrity management of patient clinical documentation in the Dayton Children’s Hospital (DCH) Electronic Health Record (EHR), otherwise known as Epic, is foundational to the care of all patients within the DCH organization. This patient database (Epic) serves as the foundation of care, patient communication and clinical coordination within the DCH network and beyond.
Under general supervision, this position is responsible for maintaining the integrity of the content of the DCH Epic electronic health record by performing in-depth investigations of questionable data entry and correcting as appropriate, researching and resolving duplicate patient records and assisting the Data Integrity Manager with other identity-related concerns. The Data Integrity Specialist provides issue identification, assessment, resolution, and technical support in order to achieve desired outcomes and compliance with DCH policies/procedures and standards.
This role analyzes chart correction scenarios and performs subsequent hands-on technical chart corrections as part of daily work responsibilities. This position serves as an organizational resource for issues involving chart correction and other identity-related concerns. The Data Integrity Specialist works under the guidance of the Data Integrity Manager at the center of the chart correction process, facilitating correction of clinical information throughout all inpatient and outpatient information systems, coordinates the parties and interdisciplinary teams involved in each case (i.e., finance, clinical departments, CIS applications, etc.), is responsible for ensuring all necessary corrections are made in a timely manner and validates each case prior to closure to ensure that documentation is complete and accurate to support patient safety and continuity of care.
Department Specific Job Details:
1. CHART CORRECTION
- Assists with identifying and resolving registration and data quality issues within the Epic EHR in order to maintain a complete and accurate legal health record for each patient.
- Assists in the development and implementation of policies and procedures to streamline the workflow of Chart Correction, increase productivity and maintain consistency with current practice.
- Utilizes the Chart Correction Tools upon patient discharge to perform the appropriate actions necessary to resolve registration and/or data quality issues in a timely manner such that it minimizes the impact to patient safety.
- Utilizes Epic Chart Correction Case tool to document scenarios requiring chart correction(s) and to track root causes, the staff involved, steps taken and resolutions.
- Maintains communication with active chart correction participants throughout the chart correction process via e-mail, phone calls and in-basket messaging.
- Acts as a contact for clinical staff for identifying data quality issues and ensures these issues are communicated to the responsible parties and addressed completely and correctly.
- Communicates with clinical staff, Epic CIS Teams and ancillary system administrators to provide detailed information on how to report and correct health record documentation.
- Monitors and replies to all communication sent to Chart Correction team within the email box and in-basket.
- Monitors, reports on and advises regarding fluctuations in Patient Overlay activity.
- Performs corrections to demographic data, such as legal name changes, erroneously entered DOB, SSN entry, etc.
- Develops and maintains relationships with a wide variety of internal and external colleagues to accomplish departmental and hospital objectives.

2. DUPLICATE RECORD MAINTENANCE
- Demonstrates an understanding of data flow between Epic and various ancillary systems. Obtains a general understanding of site level information systems both active and historical (legacy).
- Monitors and works through the Potential Duplicate Work Queues within Epic.
- Retrieves paper charts from offsite storage facility (Access) to perform chart merges.
- Maintains open communication with staff at offsite storage facility (Access) for paper charts to address discrepancies in the database and/or discrepancies related to chart indexing/identification.
- Monitors the Patient Overlay Work Queue, reports on and advises regarding fluctuations in patient overlay activity.
- Provides input/feedback to HIM CIS Application Coordinator for changes and enhancements related to HIM/Identity functionality. Works with HIM CIS Application Coordinator to review, test and implement new functionality for the EHR.
- Assists in cross-training HIM team members to assist with duplicate records.
- Adheres to policies and procedures enabling efficiency of the HIM Department.
