Physical Therapy Assistant

Summary/Objective

The Therapy Services Department provides physical, occupational, and speech therapy services to all residents at Bluegrass Hazelwood. The Therapy Services Department also works closely with the Nutritional Department in the development of programs. Staff in the department complete discipline specific assessments, perform treatments, develop mealtime and physical support plans, train staff and monitor programs. All services are provided at the highest standard of professional practice, complying with standards by both federal and state regulations and facility policies.

Essential Functions

  • Implements treatment plan of care at the direction of the physical therapist.
  • Monitor and report status and progress of individuals.
  • Complete all required documentation.
  • Train and monitor physical support plans.
  • Provide staff training as assigned.
  • Participate in interdisciplinary team meetings and other meetings as assigned.
  • Perform other duties as assigned by Therapy Director or Assistant Therapy Services Director.
  • Shall provide services to meet the treatment needs of the specialty intermediate care (IC) clinic patients for services in field of care. Shall meet requirements of 902 KAR 20:410 Specialty Intermediate Care Clinics as warranted.
  • Comply with all state physical therapy licensing requirements and regulations.
  • Complete all required inpatient and outpatient billing.
  • Learn and follow all policies and procedures.

Supervisory Responsibility

Supervises therapy support staff when needed.

Position Type/Expected Hours of Work

This is a full-time position, days and hours of work are typically Monday through Friday, 7:30-5:30. Therapy assistants are expected to flex their hours to meet the individual and facility needs. This is an hourly 37.5-hour work week.

Travel

This position may require travel to the Community Homes locations as well as community programs outside of Hazelwood Center when an individual is transitioning (ie SCL, day program, FHP, etc…). Travel is less than 10% of the expected total job duty.

Required Education and Experience

  • Graduate of an accredited physical therapist assistant program.
  • Must be licensed or license eligible by the Kentucky State Board of Physical Therapy.

Preferred Education and Experience

  • Preferred experience working with individuals with intellectual disabilities.
